Travel insurance concerns

My in-laws (both in 80's) have been travelling to Florida for past 20 winters, but, both have incurred medical issues in past few years, (ie: heart conditions). We are applying for Insurance, and realize cost will be significant, however the quality of life that they enjoy in Florida is worth the extra cost.
My question is "what happens if they develop heart problems while in Florida, will the Insurance Co. cover them?" If not, does the family run the risk of incurring exorbenent costs? If anyone has experience with this and can comment, would appreciate input.
