Johanna Booy of Care & Company provides care coordination and post rehab fitness therapy for seniors in Victoria, B.C. Her team approach to liaising with family, physicians, and community care agencies to establish care protocol for elderly and frail clients ensures her clients receive full benefits of professional full service care. Johanna offers a transitioning step, bridging the gap from hospital to home, or from independent living to assisted living and/or complete care in a care home. For most of Johanna's elderly clients the desire to 'Age in Place' is key!

As a Medical Exercise Specialist, Johanna’s goal is to restore function to perform activities of daily living for the frail or elderly seniors who have medical conditions. To that end, Johanna customizes and implements safe, effective rehabilitative fitness programs.

Care & Company bridges the gap between hospital and home for your loved one. Aging-in-place is a concept many seniors are embracing. The choice to remain at home for as long as possible is the dream of most individuals. Providing all encompassing care coordination is vital to ensure needs are met for the individual once at home. Johanna is very familiar with Victoria's community agencies for seniors.The following services are provided:

  • Initial consultation with family and health care team
  • Meet with Quick Response Team (Q RT) and VIHA Community Care Nurse
  • Coordination of caregivers and medical staff schedules including holidays
  • Development of thorough care plan
  • Arrange equipment loan or purchase, transport and delivery
  • Fall prevention assessment
  • Transportation and advocate at physician, specialist, physiotherapist appointments
  • Life skills support for activities of daily living (ADL)
  • Monthly reports of care progress and physician visits

Care & Company staff work with seniors who require development of functional strength as a result of a condition or multiple conditions. Johanna is a certified Medical Exercise Specialist having acquired her certification through the American Academy of Health, Fitness, and Rehab Professionals (AAHFRP).

Her focus is to design and implement safe and effective exercise program protocols for individuals with medical conditions such as:

  • Hip & Knee Replacements
  • Diabetes & Hypertension
  • Shoulder Impingement and Frozen Shoulder
  • Back surgery
  • Pre/post Surgery Strengthening
  • Functional strengthening for Activities of Daily Living

Johanna is a certified 'Third Age Fitness' instructor, trained in the physiological, emotional and mental requirements of the senior. The frail elderly senior returning from hospital will often require a continuation of the physiotherapy program practiced and prescribed while at hospital. Clients with hip or knee replacements are especially encouraged not only to continue their exercises in home but to build progressively to achieve maximum functional strength, flexibility and mobility. The success of the surgery is often dependent upon the exercise therapy the individual receives both prior to and post surgery.
